| Please register your contact information with the Distrcit using this utility. This information will help the District, Leagues, and Teams improve communication for our Interleague Play, TOC, and All-Star games. Your contact information will not be released outside of the District 6 Staff, your local league staff, and the other teams in your Division. If you coach in multiple Divisions, please register once for each Division. YOU MAY NOT COACH IN THE DUGOUT IF YOU ARE NOT REGISTERED AS A DISTRICT 6 COACH IN THIS DATABASE |
| INSTRUCTIONS |
| Please follow these instructions when filling out the Coaches Registration Form. |
| REGISTRATION TYPE |
| The first time (this year) you are registering as an FLD6 Coach, it is a "New Registration ". To correct a mistake on a previous registration, or if your contact information has changed, please file a "Change or Correction". |
| SEASON |
| Indicate the season of play for the game you are coaching. Regular Season Interleague Play (RegS) -or- Tournament of Champions (TOC) -or- All-Stars |
| DIVISION |
| Indicate the division of play for the game you are reporting. |
| LEAGUE |
| Please Indicate which FLD6 league you are from. If you are not from a FLD6 league, type the District & League Name in "Other League" field. |
| TEAM INFORMATION |
| List your Team's Name (e.g. Rays, Yankees, Thrashers, etc.). If your league uses Team Numbers, please indicate your Team Number. |
| COACHING TITLE |
| Please indicate if your are the team's Manager, Coach, or Coordinator. |
| CONTACT INFORMATION |
| Please give us your contact information for communication with your team. The District Staff, your local league's staff, and the other teams you play may need to get in touch with you to cancel or resceudule a game, or give you other important information. Please fill this inforamtion out as completely as possible. |

| Let us know if you would like to record any additional comments regarding your registaton. |
| SUBMIT REGISTRATION |
| Click-on "Submit Registration " to record your contact informationto the District Coaches Database. If you would like to clear this form of all data you have entered and NOT record the data, click-on "Clear Form". If you have submitted a registration to the database and discover that there were errors in the data or your information changes at any time, please re-submit your contact information as a "Change or Correction" (see "RegistrationType" above). |
